Bringing advanced wound medicine home to support healing where patients live

Integrity Wound Healing Solutions, LLC was founded from a passion for expanding access to specialty wound management, especially for patients whose mobility, frailty, or medical complexity can make traditional clinic-based care difficult.

By providing wound management directly to patients in their homes and residential communities, we help reduce barriers to care while supporting independence, continuity, and aging in place. Our approach is evidence-based, collaborative, and centered on the whole person, with patients, caregivers, families, and care teams working together toward a shared plan for healing.

Our Story

Specialty wound management, without the burden of travel to a clinic

A wound clinic appointment may sound simple, but for an older adult or a patient with limited mobility, getting there can become an undertaking of its own. Transportation, caregiver availability, mobility limitations, oxygen needs, cognitive impairment, and the physical demands of leaving home can all create barriers to consistent follow-up.

When access to care becomes difficult, wound management can become fragmented by missed visits. Gaps in care can lead to worsening of the wound condition or stalled progress.

Integrity was designed around that reality. The model is simple: the clinician travels, not the patient. Assessment, treatment, debridement when appropriate, compression, offloading, education, and follow-up are provided where the patient lives, with caregivers, family members, facility staff, and the broader care team included in the plan whenever appropriate.

Natalie is a board-certified family nurse practitioner with advanced specialty certification in wound healing. Her vision for Integrity Wound Healing Solutions grew from a passion for expanding access to care and supporting patients where they live. She brings an evidence-based, whole-person approach to each visit, partnering with patients, caregivers, families, and care teams to develop an individualized plan of care. Natalie also serves on the Advisory Board of the Academy of Prescribers in Wound Healing, contributing to education and advancement within the wound management community.

What the letters mean. MSN — Master of Science in Nursing. APRN — Advanced Practice Registered Nurse. FNP-C — Family Nurse Practitioner, board certified. DAPWHc — Diplomate of the Academy of Prescribers in Wound Healing. CWHAP — Certified Wound Healing Advanced Practitioner, a specialty credential open only to advanced practice clinicians with documented wound experience.

What to Expect

Your first visit, step by step

1. Referral

A patient, family member, caregiver, provider, case manager, discharge planner, home health agency, or facility may initiate a referral. We verify insurance information and coordinate with your PCP to request the records needed to prepare for your first visit.

2. Scheduling

We schedule the first visit within 24–72 business hours, at a time that works for you and your caregiver whenever possible.

3. Full assessment

A typical visit includes wound measurement and photography, circulation and pressure screening, nutrition and risk assessment, and a review of current treatments, medications, and other factors that may be affecting healing — followed by treatment.

4. Plan and teaching

We partner with you and your caregiver to develop a clear plan of care, identify the appropriate supplies and treatments, provide customized patient education, and establish a schedule for follow-up.
